Case Manager

Rafiki Coalition seeks a Case Manager/Social Worker to support clients engaged in marine-focused and community wellness programs in partnership with OceanTech. In this role, you will conduct assessments, develop individualized service plans, and coordinate referrals to health, housing, and social services. You will collaborate closely with researchers and technical teams to integrate client needs into innovative, sustainability-driven projects. The ideal candidate brings strong clinical judgment, experience with diverse communities, excellent documentation skills, and a passion for environmental and social justice in a collaborative, growth-oriented culture.

Responsibilities

• Provide case management and social work support to clients engaged in marine-related community and conservation programs
• Conduct client assessments, develop service plans, and coordinate resources and referrals
• Collaborate with multidisciplinary research and technical teams to align client services with project goals
• Maintain accurate documentation, case notes, and reports in compliance with organizational standards
• Advocate for client needs and connect individuals and families to community, healthcare, and social services
• Monitor client progress, adjust plans, and provide crisis intervention as needed
• Participate in program evaluation, data collection, and continuous improvement initiatives
• Support outreach, education, and workshops related to environmental health and community well-being
