Pediatric High-Acuity Home Health Licensed Vocational Nurse

Aveanna Healthcare is seeking a Pediatric LVN for high-acuity home health care. In this role, you'll provide skilled nursing to medically fragile children in their homes, including assessments, medication administration, ventilation and tracheostomy care, and implementation of care plans. You'll collaborate with families and interdisciplinary teams to promote optimal outcomes and safety. Responsibilities

• Provide skilled nursing care to high-acuity pediatric patients in the home setting
• Perform comprehensive assessments and ongoing monitoring of patient status
• Administer medications and treatments per physician orders
• Manage ventilators, tracheostomies, and other complex equipment safely
• Implement and document individualized care plans using EMR systems
• Educate and support patients' families on care techniques and safety
• Collaborate with interdisciplinary clinical teams to coordinate care
• Maintain infection control and safety standards in the home environment
• Communicate changes in patient condition promptly to physicians and supervisors
• Participate in ongoing training and competency development
