- Licenses: Active, unrestricted Indiana Medical License (or ability to obtain); Indiana Controlled Substance Registration; DEA with Indiana address and schedules 2, 2N, 3, 3N, and 4
- DEA / CDS: Active DEA with Indiana address and Indiana Controlled Substance Registration required
- Certifications: BLS, ACLS, and PALS required
- Experience: Completion of ACGME-accredited Emergency Medicine Residency; Emergency Department experience preferred
- Other Qualifications: Case logs covering previous two years; CME Attestation Form; current malpractice coverage history with 10-year claims history; current Flu vaccination (during active flu season); TB test within previous 12 months (or chest x-ray for positive history); COVID vaccination documentation
- Provide comprehensive emergency medical care to patients at Franciscan Health Munster and Franciscan Health Dyer.
- Manage moderate acuity cases in a 63 bed acute care hospital with approximately 15,000 annual ED visits.
- Perform moderate sedation as required.
- Collaborate with nursing staff and support personnel to ensure efficient patient flow.
- Maintain cross credentialing and adhere to all facility protocols and compliance requirements.
