Director of Social Services - Long-Term Care

Marmet Center, a Genesis HealthCare skilled nursing and rehabilitation facility, seeks a Social Services Director to lead resident-centered psychosocial care. This role oversees assessments, care planning, discharge coordination, and family support while ensuring regulatory compliance. You will collaborate closely with nursing, therapy, and medical teams, provide counseling and advocacy, and connect residents with community resources. The director mentors social services staff, supports a culture of dignity, respect, and safety, and plays a key role in enhancing quality of life for residents and their families.

Responsibilities

• Lead and manage social services programs to support residents and families in a skilled nursing and rehab setting
• Conduct psychosocial assessments and develop individualized care plans aligned with resident goals
• Participate in interdisciplinary care conferences and coordinate transitions of care and discharge planning
• Provide counseling, crisis intervention, and advocacy for residents and their families
• Ensure compliance with federal, state, and facility regulations and documentation standards
• Supervise and mentor social services staff, fostering a collaborative, resident-centered culture
• Serve as liaison between residents, families, external agencies, and facility leadership
• Organize and support family meetings, education, and community resource referrals
