Primary Care Physician

Amergis seeks a mission-driven Primary Care Physician to deliver comprehensive outpatient care across diverse communities. In this role, you will perform assessments, diagnose and manage acute and chronic conditions, coordinate preventive care, and develop evidence-based treatment plans. Collaborate with nurses, behavioral health, and allied staff across schools, homes, and facilities. Uphold rigorous compliance, documentation, and quality standards while mentoring staff and participating in quality improvement. Responsibilities

• Provide comprehensive primary care, including assessment, diagnosis, and management of acute and chronic conditions.
• Develop and implement evidence-based treatment and preventive care plans.
• Order, interpret, and act on diagnostic tests and imaging.
• Document care accurately and timely in the Electronic Health Record (EHR).
• Coordinate care with nurses, behavioral health, and allied health professionals across settings.
• Ensure compliance with regulatory, payer, and organizational quality standards.
• Educate patients and families on conditions, medications, and lifestyle changes.
• Participate in quality improvement, population health, and care management initiatives.
• Contribute to a collaborative, mission-driven, and culturally competent care environment.
• Provide occasional mentorship or guidance to clinical staff and learners as appropriate.
