Home Care Registered Nurse (RN) - Job Description
We are seeking a compassionate and experienced Home Care Registered Nurse (RN) to join our team. The ideal candidate will provide high-quality, patient-centered care in a home setting and play a key role in improving patients' health outcomes and quality of life.
Responsibilities:
Provide skilled nursing care to patients in their homes
Assess patient conditions and develop individualized care plans
Monitor and document changes in patient health, symptoms, and behavior
Administer medications and treatments as prescribed
Collaborate with physicians and other healthcare professionals to coordinate care
Educate patients and families on health maintenance and disease prevention
Facilitate referrals to specialists and community resources as needed
Maintain accurate and timely medical records
Offer emotional support and guidance to patients and their families
Qualifications:
Active Registered Nurse (RN) license in the state of Virginia
Previous nursing experience required; home care experience preferred
Strong clinical assessment and critical thinking skills
Familiarity with electronic medical records and medical equipment
Excellent communication and interpersonal skills
Ability to build rapport with patients and families
Ability to work independently and manage time effectively in a home care setting
