RN Care Manager

Intermountain Health is seeking an RN Care Manager to coordinate patient care across settings in our mission-driven, team-based environment. Responsibilities

• Perform comprehensive nursing assessments and develop individualized care plans
• Coordinate care transitions and discharge planning across care settings
• Collaborate with physicians, social workers, and interdisciplinary teams to optimize outcomes
• Monitor patient progress, adjust care plans, and address barriers to care
• Educate patients and families on diagnoses, treatments, and self-management
• Ensure appropriate utilization of healthcare resources and services
• Document care activities accurately in the electronic health record (EHR)
• Support quality improvement and population health initiatives within the system
