Community Based Counselor

Aspire Health Partners, a leading nonprofit behavioral healthcare provider in Central Florida, is seeking a Community Based Counselor to deliver client-centered counseling and recovery-focused support in home and community settings. In this role, you will provide mental health and substance use assessments, facilitate individualized treatment and safety plans, and deliver evidence-based interventions for children, adults, and families. You will coordinate care with multidisciplinary teams, document in EHR systems, and link clients to community resources. Responsibilities

• Provide community-based counseling and support to clients with mental health and substance use needs
• Conduct biopsychosocial assessments, risk evaluations, and safety planning
• Develop and implement individualized, recovery-focused treatment plans
• Deliver evidence-based interventions for individuals and families in home and community settings
• Coordinate care with psychiatrists, nurses, case managers, and community partners
• Maintain accurate, timely documentation in electronic health records (EHR)
• Educate clients and families on coping strategies, recovery, and available resources
• Facilitate referrals and linkages to community services and supports
• Participate in team meetings, supervision, and ongoing training
• Uphold Aspire's mission, cultural values, and regulatory and ethical standards
