Job Description

Martin Luther King, Jr. Community Hospital seeks an RN Care Manager for our Emergency Room to coordinate patient care from arrival through safe discharge. In this role, you will perform thorough assessments, develop and update care plans, and collaborate closely with physicians, social workers, and the interdisciplinary team to ensure timely, appropriate services and smooth transitions of care. You will educate patients and families, advocate for health equity, and connect individuals with community resources that support recovery and long-term wellness. The RN Care Manager monitors utilization, quality, and safety indicators while maintaining accurate documentation in the EHR. You will participate in huddles, case conferences, and performance improvement initiatives focused on emergency throughput and patient outcomes. Responsibilities

  • Coordinate and manage care for emergency room patients from admission through discharge
  • Conduct nursing assessments, develop individualized care plans, and update them based on patient status
  • Collaborate with physicians, social workers, and interdisciplinary teams to ensure seamless transitions of care
  • Educate patients and families on diagnoses, medications, discharge plans, and community resources
  • Advocate for patients' needs, focusing on health equity and culturally responsive care
  • Monitor quality, safety, and utilization metrics, escalating concerns as needed
  • Document assessments, interventions, and care plans accurately in the electronic health record
  • Support process improvements that enhance emergency department throughput and patient outcomes
  • Participate in staff huddles, case conferences, and ongoing clinical education
  • Ensure compliance with hospital policies, regulatory standards, and best-practice guidelines
