Job Description
Optimal Care is seeking a compassionate Home Health Registered Nurse to provide skilled nursing care to patients in their homes. In this role, you will perform comprehensive assessments, develop and update care plans, administer medications and treatments, and educate patients and families on disease management and safety. You will coordinate with physicians, therapists, and caregivers, document in our electronic health record, and manage a mobile caseload. The ideal candidate is a self-directed RN with strong clinical judgment, excellent communication skills, and reliable transportation.
Responsibilities
- Perform comprehensive in-home nursing assessments and ongoing evaluations
- Develop, implement, and update individualized care plans
- Administer medications, treatments, wound care, and IV therapies as ordered
- Educate patients and families on disease management, medications, and home safety
- Coordinate care with physicians, therapists, and other healthcare providers
- Accurately document all care and communications in the electronic health record
- Monitor patient progress, recognize changes in condition, and report promptly
- Promote infection control and safe practices in the home environment
- Manage a home-visiting schedule and travel between patient residences
- Participate in case conferences and quality improvement activities
