Geriatric Home Health Physical Therapist

FOX Rehabilitation seeks a Physical Therapist to provide evidence-based geriatric care in patients' homes and senior living communities. Evaluate, develop, and deliver functional treatment plans focused on mobility, fall prevention, and quality of life. Responsibilities

• Provide in-home and senior living physical therapy evaluations and treatments for older adults
• Develop, implement, and adjust individualized, evidence-based treatment plans
• Focus on functional mobility, balance, fall prevention, and improved quality of life
• Document care accurately and timely in EMR systems
• Collaborate with an interdisciplinary team and communicate with caregivers and referring providers
• Educate patients and caregivers on home exercise programs and safety
• Participate in ongoing clinical education and mentorship opportunities
• Contribute to a mission-driven, patient-first culture and productivity goals
