Social Services Director

Holly Manor Center, a Genesis HealthCare skilled nursing and rehabilitation facility in Hackettstown, NJ, seeks a Social Services Director to lead resident-centered psychosocial care. This role oversees social services delivery, assessments, care planning, discharge planning, and coordination of community resources for short-term rehab and long-term care residents. Responsibilities

• Lead and manage the social services department within the skilled nursing facility
