Healthcare Social Work Care Manager

Intermountain Health is seeking a Social Work Care Manager to support patients across care settings in Salt Lake City. Responsibilities

• Conduct comprehensive biopsychosocial assessments for patients across care settings
• Develop, implement, and regularly update individualized care plans in collaboration with patients, families, and providers
• Coordinate community resources, social services, and benefits to address social determinants of health
• Provide crisis intervention, brief counseling, and supportive case management
• Facilitate care transitions between hospital, clinic, home, and community agencies
• Advocate for patient rights, access to services, and culturally competent care
• Collaborate with interdisciplinary care teams to ensure integrated, patient-centered care
• Monitor patient progress, document interventions, and track outcomes in the electronic health record
• Educate patients and families on treatment plans, resources, and self-management strategies
• Participate in quality improvement, population health, and community health initiatives
