Peer Navigator

The Peer Navigator provides counseling, advocacy and health-navigation support to people living with HIV/AIDS and other underserved populations. Using relevant personal experience, the navigator helps members access healthcare, recovery resources, community services and housing, while documenting progress and coordinating with clinical teams.

Responsibilities

• Develop individualized action plans and guide members through the healthcare system.
• Provide peer support, counseling, mentoring and education on self-help and coping skills.
• Connect members with primary care, substance-use treatment, preventive care and community support services.
• Conduct outreach and follow-up, and accompany members to medical appointments as needed.
• Advocate for members' recovery goals, housing transitions and community support networks.
• Document client interactions and progress, and participate in case conferences, meetings and training.
