Physical Therapist - Home Health

Adventist Health is seeking a full-time Home Health Physical Therapist to provide compassionate, evidence-based rehabilitation for patients in their homes. In this mission-driven, faith-based role, you will perform comprehensive evaluations, develop and adjust individualized care plans, deliver therapeutic interventions, educate patients and caregivers, and collaborate closely with nurses, physicians, and other team members. You'll support safe mobility, pain reduction, and functional independence while documenting accurately, meeting regulatory standards, and contributing to whole-person care-mind, body, and spirit.

Responsibilities

• Provide in-home physical therapy evaluations and treatments
• Develop and update individualized plan of care and goals
• Implement therapeutic exercises, gait and balance training, and functional mobility interventions
• Educate patients and caregivers on home exercise programs and safety
• Collaborate with nurses, physicians, and interdisciplinary team members
• Document visits, progress notes, and outcomes in the EMR accurately and timely
• Ensure compliance with home health, regulatory, and payer requirements
• Assess need for and recommend durable medical equipment and adaptive devices
• Promote whole-person, faith-aligned care, respecting patient values and culture
• Participate in case conferences, quality initiatives, and professional development
