Community Health Outreach Representative I

Oak Street Health seeks an Outreach Executive I to connect Medicare-eligible adults in underserved communities with our innovative primary care centers. In this patient-centered role, you will conduct community-based and phone outreach, educate prospective patients on preventative care, and assist with scheduling and onboarding. Working closely with clinical and care teams, you'll help reduce barriers to access, support retention, and maintain accurate engagement records. Responsibilities

• Engage Medicare-eligible adults in underserved communities to introduce Oak Street Health's primary care services
• Conduct outbound and in-person outreach to educate patients on preventative care and clinic benefits
• Support patients with appointment scheduling, reminders, and follow-up to reduce no-show rates
• Partner with clinical and care teams to coordinate seamless patient onboarding
• Track outreach activities, maintain accurate records, and report on engagement metrics
• Represent Oak Street Health at community events, health fairs, and local partner sites
• Provide exceptional customer service and resolve basic patient questions or concerns
• Continuously learn about Oak Street Health programs to clearly explain value to patients
