Seeking an experienced Nurse Practitioner to join a collaborative primary care team in Chowchilla, CA. This is a full-time opportunity providing comprehensive family and preventive care to a diverse community population.

• Provide comprehensive primary and preventive care
• Evaluate, diagnose, and treat acute and chronic conditions
• Develop treatment plans and coordinate referrals and follow-up
• Perform appropriate office-based procedures
• Maintain accurate clinical documentation
• Collaborate with physicians and other healthcare professionals
• Participate in quality improvement and community health initiatives

• Graduate of an accredited Nurse Practitioner program
• Active California NP license or eligibility
• 12+ months of clinical experience preferred
• Primary care/family practice experience preferred
• Strong communication and patient-care skills

• $150,000 $212,000 annually
