Community Support Specialist - Long-Term Care

CommuniCare is seeking a Community Support Specialist to enhance resident well-being across our post-acute and long-term care settings. In this role, you will assess psychosocial needs, coordinate community and social services, and support residents and families through care transitions. You'll collaborate closely with nursing, therapy, and leadership teams to create resident-centered plans, connect individuals with resources, and promote engagement in programs. Responsibilities

• Serve as primary contact for residents and families to coordinate social and support services
• Assess psychosocial needs and develop person-centered support plans
• Connect residents with community resources, benefits, and advocacy services
• Collaborate with nursing, therapy, and administrative teams to support care goals
• Facilitate care conferences, educational sessions, and family meetings
• Document assessments, interventions, and outcomes in accordance with regulations
• Support transitions of care, including admissions, discharges, and referrals
• Promote resident rights, dignity, and engagement in facility programs
