Job Description
Sevita is seeking a Program Specialist to support community-based services that empower individuals of all ages and abilities. In this role, you'll coordinate and monitor programs, maintain documentation, and collaborate with interdisciplinary teams to ensure person-centered care. You will engage with individuals and families, connect them to resources, and support goal planning and progress tracking. Responsibilities
- Manage a caseload of individuals receiving community-based services and supports.
- Conduct intake interviews and comprehensive needs assessments for new participants and families.
- Develop and update person-centered service or care plans in collaboration with individuals, families, and interdisciplinary team members.
- Coordinate and monitor delivery of services such as behavioral health support, rehabilitation, and specialized foster care.
- Maintain accurate, timely documentation, including case notes, service authorizations, and outcome reports.
- Communicate regularly with clinical staff, direct support professionals, and external providers to coordinate care.
- Advocate for participants to access needed resources, benefits, and accommodations in the community.
- Build and maintain partnerships with local agencies, schools, healthcare providers, and other community organizations.
- Participate in community outreach, informational sessions, and events to connect individuals with Sevita services.
- Use electronic case management or health record systems to track services, progress, and compliance with regulations.
- Respond to crises or urgent needs by coordinating appropriate supports and following established protocols.
- Participate in ongoing training, team meetings, and quality improvement initiatives to enhance program effectiveness.
