Permanent Staff LMSW

Responsibilities

• Provide psychosocial assessments for patients and families
• Develop and implement discharge and transition plans
• Coordinate community resources, benefits, and referrals
• Deliver crisis intervention and short-term counseling
• Collaborate with physicians, nurses, and care teams to optimize patient outcomes
• Document assessments, plans, and interventions in the EHR accurately and timely
• Advocate for patients' social, emotional, and financial needs
• Ensure compliance with hospital policies and regulatory standards
• Participate in interdisciplinary rounds and care conferences
• Contribute to quality improvement and best-practice initiatives
